No. If you're just doing the standard circuits, sturdy walking shoes or trail runners are fine. Boots only matter if you're doing the Inca Trail or similar.

Yes, but keep it small and comfortable. The exact limits depend on the train operator, service, and current site rules, so check your ticket and the official information before travelling.

What's the one thing I'd buy that most people skip?

A proper dry bag that fits inside your pack. Will save your phone, passport, and dignity in a downpour.

How many days should I pack for?

One set of hiking clothes, one set of evening clothes. Laundry is cheap in Aguas Calientes. Pack light, wash on the road.

Is bug spray actually necessary?

We didn't need it at Machu Picchu itself. If you're going into the Amazon side (Puerto Maldonado), absolutely. For the citadel, your call.
